[Abstract] Alzheimer's disease (AD) is a neurodegenerative disorder characterized by progressive cognitive impairment and represents the most common cause of dementia. Acupuncture has been used as a therapeutic approach for AD and has shown potential in alleviating AD-related symptoms and delaying disease progression. Neuroimaging techniques provide objective and quantifiable evidence for elucidating the central mechanisms underlying acupuncture effects in AD. However, existing studies have predominantly focused on functional magnetic resonance imaging, and a systematic synthesis and comprehensive analysis from a multimodal neuroimaging perspective remain lacking regarding the objective effects of acupuncture in the treatment of AD, the key brain regions involved, the patterns of brain network modulation, and the underlying mechanisms. Therefore, we systematically summarize neuroimaging advances in acupuncture intervention for AD, focusing on magnetic resonance imaging modalities, including structural MRI, functional MRI, diffusion tensor imaging, arterial spin labeling, and magnetic resonance spectroscopy, as well as positron emission tomography and functional near-infrared spectroscopy. We synthesize findings related to brain structure, functional activity and connectivity, cerebral metabolism, and cerebral blood flow/perfusion, and discuss AD-related intracerebral alterations and the putative central regulatory mechanisms of acupuncture from a multidimensional perspective. This review aims to provide a reference for future studies and to facilitate further mechanistic and clinical research in this field.
