[Abstract] Imaging assessment of inflammatory bowel disease (IBD) is shifting from simple lesion detection toward quantitative evaluation of disease activity, differentiation between inflammatory and fibrotic strictures, prediction of treatment response, and long-term risk stratification. Intestinal ultrasound (IUS), computed tomography enterography (CTE), and magnetic resonance enterography (MRE) are the core modalities for multimodal imaging assessment of IBD. However, conventional image interpretation still relies heavily on physicians' experience, leading to subjectivity, limited inter-observer consistency, and insufficient use of high-dimensional image information. Deep learning and radiomics provide new technical routes for quantitative IBD imaging through automatic segmentation, feature extraction, and predictive modeling. This review summarizes the application progress of deep learning and radiomics in IUS, CTE, and MRE, and compares the performance, advantages, and limitations of different models across key clinical tasks, including disease activity quantification, stricture characterization, complication assessment, treatment response prediction, and prognosis prediction. Current bottlenecks related to data standardization, external validation, interpretability, and clinical system integration are further discussed. Future studies should strengthen multicenter prospective validation, real-time computer-aided diagnosis, multimodal and multi-omics integration, and image-pathology correspondence research, thereby facilitating clinical translation and providing references for clinicians and researchers in selecting appropriate radiomics and deep learning approaches for IBD.
