[摘要] 目的 探讨 2 型糖尿病相关认知障碍(type 2 diabetes mellitus-associated cognitive dysfunction, TDACD)患者的大脑结构和功能变化特点,分析其可能的神经机制。材料与方法 本研究已在国际注册前瞻性系统评价研究(Prospective Register of Systematic Reviews, PROSPERO)平台上注册,注册号为CRD420251115489。按 PRISMA 流程检索并纳入全脑基于体素的形态学分析(voxel-based morphometry, VBM)与静息态功能磁共振成像(resting-state functional magnetic resonance imaging, rs-fMRI)研究,采用基于种子点差异映射和主题图像排列(seed-based d mapping with permutation of subject images, SDM-PSI)进行灰质体积(gray matter volume, GMV)与静息态功能连接(resting-state functional connectivity, rs-FC)荟萃分析;进行敏感性、异质性与发表偏倚检验,并以人口统计学和临床变量[年龄、性别、身体质量指数(body mass index, BMI)、受教育年限、病程、糖化血红蛋白(glycated hemoglobin A1c, HbA1c)、简易智力状态检查量表(Mini-Mental State Examination, MMSE)、蒙特利尔认知评估量表(Montreal Cognitive Assessment, MoCA)]进行元回归分析。结果 共纳入12项研究(功能 7 项、结构 5 项)。与健康对照(healthy control, HC)相比,TDACD 患者在左缘上回(supramarginal gyrus, SMG)[蒙特利尔神经研究所(Montreal Neurological Institute, MNI)坐标:(-54,-54,28),SDM-Z=-4.678,P<0.000 5)与右眶额回(inferior frontal gyrus, orbital part, IFGorb)[MNI 坐标:(48,40,-4),SDM-Z=-3.649,P<0.001]存在脑功能活动降低,右侧壳核(putamen, PUT)[MNI坐标:(34,-4,4),SDM-Z=-2.933,P<0.01] GMV减少;敏感性分析结果稳定。元回归分析显示教育年限越高,右IFGorb功能下降越轻(r=0.558,P<0.005);其余变量无显著调节效应。结论 TDACD 患者存在皮层—皮层下环路的结构与功能异常,尤其是以PUT-SMG-IFGorb为核心的相关脑区受损可能构成潜在的神经影像标志。这些结果为TDACD的病理机制、早期识别及神经调控干预提供重要线索。
[Abstract] Objective To investigate structural and functional brain alterations in type 2 diabetes mellitus-associated cognitive dysfunction (TDACD) and to explore potential neural mechanisms.Materials and Methods This study has been registered on the Prospective Register of Systematic Reviews (PROSPERO) with the registration number CRD420251115489. Whole-brain voxel-based morphometry (VBM) and resting-state functional magnetic resonance imaging (rs-fMRI) studies were systematically identified and selected according to PRISMA guidelines. A meta-analysis of gray matter volume (GMV) and resting-state functional connectivity (rs-FC) was conducted using seed-based d mapping with permutation of subject images (SDM-PSI), together with sensitivity, heterogeneity, and publication bias testing. Meta-regression analyses were additionally conducted using demographic and clinical variables [age, sex, body mass index (BMI), education, disease duration, glycated hemoglobin A1c (HbA1c), Mini-Mental State Examination (MMSE), Montreal Cognitive Assessment (MoCA)].Results Twelve studies (7 functional and 5 structural) were included. Compared with healthy controls (HC), patients with TDACD demonstrated reduced functional activity in the left supramarginal gyrus [SMG, MNI coordinates: (-54, -54, 28), SDM-Z = -4.678, P < 0.000 5] and right inferior frontal gyrus, orbital part [IFGorb, MNI coordinates: (48, 40, -4), SDM-Z = -3.649, P < 0.001)], accompanied by significant gray matter volume reduction in the right putamen [PUT, MNI coordinates: (34, -4, 4), SDM-Z = -2.933, P < 0.01]. Sensitivity analyses confirmed the robustness of these findings. Higher levels of education were associated with milder functional impairment in the right IFGorb (r = 0.558, P < 0.005), whereas no other variables showed significant moderating effects.Conclusions Patients with TDACD exhibit structural and functional abnormalities in the cortical-subcortical circuits; in particular, damage to core brain regions centered on the PUT-SMG-IFGorb may constitute a potential neuroimaging biomarker. These findings provide important clues for the pathological mechanisms, early identification, and neuromodulation interventions of TDACD.

0 引言

       2型糖尿病(type 2 diabetes mellitus, T2DM)为常见慢性代谢性疾病,全球患病率持续上升[1]。长期高血糖可导致血管和神经损伤[2, 3],并累及中枢神经系统。T2DM是认知障碍的重要危险因素,患者常见记忆、注意与执行功能受损[4]。在疾病进展中,由代谢紊乱、血管病变和神经炎症等因素引起的认知下降被称为2型糖尿病相关认知障碍(type 2 diabetes mellitus-associated cognitive dysfunction, TDACD),涉及学习、记忆、执行等多方面损害[5],并增加痴呆风险[6],影响患者的生活质量与治疗依从性。

       神经影像学的发展为理解TDACD的神经机制提供了关键工具。脑结构方面,基于体素的形态学分析(voxel-based morphometry, VBM)广泛用于评估灰质体积(gray matter volume, GMV)变化[7]。多项VBM研究显示,TDACD患者存在广泛的灰质异常[8, 9, 10]。例如,ZHANG等报告伴有轻度认知障碍(mild cognitive impairment, MCI)的T2DM患者内侧颞叶体积减少[11];LI等指出皮层及皮下灰质萎缩与认知损伤程度相关[12]。MA等[13]与HE等[14]的meta分析亦证实TDACD患者多个脑区的GMV异常,提示灰质萎缩可能是其神经退行性变的结构基础。然而,不同研究关于颞叶结构改变的结果并不完全一致。部分研究观察到海马、海马旁回及颞叶皮层等区域存在明显萎缩[12, 15],而另一些研究,如CHRIS等[16]的研究,则未发现显著的颞叶灰质异常。这种差异可能与纳入样本量、疾病严重程度、认知障碍定义标准、MRI采集参数以及分析方法学异质性等因素有关。脑功能层面,静息态功能磁共振成像(resting-state functional magnetic resonance imaging, rs-fMRI)揭示脑区间的自发活动与功能连接(functional connectivity, FC)[17]。多项研究表明,TDACD患者在注意、记忆和视觉网络中表现出FC改变。ZHANG等发现T2DM患者腹侧感觉运动网络与背侧注意网络、侧视觉网络之间的FC降低,并与认知下降相关[18];HUANG等发现MCI伴T2DM患者在右海马旁回、后扣带回和左额上回等核心节点的度中心性下降,提示视觉与记忆网络整合受损[19]。总体而言,rs-fMRI研究揭示TDACD患者大脑网络的功能重组与其认知缺陷密切相关。

       尽管神经影像研究不断积累,目前关于TDACD的脑改变仍不一致[20, 21, 22],主要原因包括:第一,样本量小,易受个体差异影响[23];第二,临床异质性高,如病程、合并症、血糖控制等均可调节脑结构与功能的代偿模式[24, 25];第三,方法学差异,例如功能连接分析中种子点选择、全脑与感兴趣区(region of interest, ROI)分析等方式不同,影响结果间的可比性[26, 27]。因此,单一研究难以系统揭示TDACD稳定的大脑改变模式[14],有必要对 TDACD患者进行meta分析,以全面研究他们的功能活动和GMV变化。本研究采用基于种子点差异映射和主题图像排列(seed-based d mapping with permutation of subject images, SDM-PSI)方法对结构(VBM)与功能(rs-fMRI)两种模态的脑影像数据进行整合分析,该方法能够在统一标准下识别TDACD患者GMV与功能活动的一致性异常脑区,减小样本与方法差异带来的偏倚,并通过元回归分析探讨临床变量对脑改变的调节作用,为理解TDACD的神经基础提供更系统、可靠的影像学证据。

1 材料与方法

1.1 方案注册

       本研究已在国际注册前瞻性系统评价研究(Prospective Register of Systematic Reviews, PROSPERO)平台上注册,注册号为CRD420251115489。

1.2 纳入与排除标准

       纳入标准:(1)同行评议原创研究(通过检索同行评议期刊发表的原创研究论文进行确认,所有纳入文献均发表于经过同行评审的学术期刊,且标注有原创研究标识);(2)研究对象为 TDACD患者与健康对照(healthy control, HC);(3)采用全脑VBM或rs-fMRI技术,并报告蒙特利尔神经研究所(Montreal Neurological Institute, MNI)坐标;(4)所有受试者均接受标准化认知评估,且TDACD 组认知功能显著低于 HC 组[认知功能低下判定标准:简易智力状态检查量表(Mini-Mental State Examination, MMSE)≤26 分或蒙特利尔认知评估量表(Montreal Cognitive Assessment, MoCA)≤22分,或采用其他标准化神经心理学评估工具且明确报告两组间认知功能差异有统计学意义]。

       排除标准:(1)综述、摘要、动物实验或未发表研究;(2)伴有卒中、脑外伤、精神疾病、酗酒及其他可能影响认知功能的神经系统疾病。

1.3 文献检索策略

       本研究遵循PRISMA声明标准。系统检索PubMed、Web of Science、Embase、中国知网(CNKI)和万方数据库,收集自建库至2025年5月31日的相关文献。检索策略结合主题词与自由词。

       中文检索式:(“糖尿病,2型” OR “2型糖尿病” OR “T2DM”)AND(“认知功能障碍” OR “认知功能下降” OR “认知障碍” OR “轻度认知障碍” OR “MCI”)AND(“脑功能连接” OR “功能磁共振成像” OR “fMRI” OR “静息态” OR “GMV” OR “体素形态测量”)。

       英文检索式:("type 2 diabetes mellitus"[MeSH Terms] OR "T2DM" OR "diabetes mellitus type 2")AND("cognitive impairment"[MeSH Terms] OR "cognitive decline" OR "mild cognitive impairment" OR "MCI" OR "cognitive dysfunction")AND("functional connectivity"[MeSH Terms] OR "resting-state fMRI" OR "rs-fMRI" OR "gray matter volume" OR "GMV" OR "voxel-based morphometry" OR "VBM")。

       本研究文献筛选与数据提取由3名研究者独立完成,包括孙志远(博士研究生,讲师,从事运动促进脑健康研究4年)、杨常智(硕士研究生,助教,从事运动促进脑健康研究2年)、许振洋(硕士研究生,助教,从事运动促进脑健康研究2年)。分歧经双方讨论或咨询通信作者(毛德伟教授、田雪文教授)协商解决。另手动检索了已纳入文献及相关综述的参考文献列表。

1.4 文献质量评估

       参考DAI等[28]的20分量表,从样本特征(10分)与影像学方法(10分)评估,具体如下。(1)样本特征:采用标准化的疾病诊断标准(1分);报告重要人口统计学数据(如年龄、性别),并提供均值与标准差(2分);明确排除精神类或其他重大系统性疾病(1分);报告关键临床指标(如病程、糖化血红蛋白、身体质量指数),并提供均值与标准差(4分);各组样本量均不少于10例(2分)。(2)影像学方法:采用全脑自动化分析流程(3分);磁场强度不低于1.5 T(1分);静息态数据采集时间不少于5分钟(1分);扫描范围覆盖全脑(1分);清晰描述数据采集与预处理步骤(1分);结果在标准空间(如MNI)中报告坐标(1分);进行多重比较校正(1分);结论与结果一致,并明确说明研究局限性(1分)。本研究参考神经影像荟萃分析的通用标准,将得分≥17分的研究定义为高质量研究。最终纳入的12项研究得分均≥17分,表明纳入研究的方法学质量整体较高。

1.5 数据提取

       两名研究者孙志远(博士研究生,讲师,从事运动促进脑健康研究4年)和杨常智(硕士研究生,助教,从事运动促进脑健康研究2年)独立提取数据,并使用预先设计的数据提取表进行信息收集。提取内容主要包括:(1)研究基本信息与受试者特征:第一作者、发表年份、各组样本量、性别比例、年龄、受教育年限、糖尿病病程、糖化血红蛋白(glycated hemoglobin A1c, HbA1c)、身体质量指数(body mass index, BMI)及认知评估工具与得分。(2)神经影像学数据:在全脑水平上,组间比较具有统计学差异的脑区峰值坐标(MNI空间)。所有提取结果由双方交叉核对。如遇分歧,通过共同复核原文或与第三位研究者许振洋(硕士研究生,助教,从事运动促进脑健康研究 2 年)协商达成一致。

1.6 结构和功能改变的荟萃分析

       采用SDM-PSI(版本 6.2.3,https://www.sdmproject.com/)对 TDACD与HC在GMV与静息态功能连接方面的差异进行分析。对于每项纳入的 VBM 或 rs-fMRI 研究,SDM-PSI 基于其报告的峰值坐标与效应量,使用各向异性的高斯核重建全脑效应大小图及方差图[29]

       具体分析流程如下:首先,从原始研究中提取TDACD与HC之间差异有统计学意义的峰值坐标及其效应量。若无t值,则通过SDM官网工具(https://www.sdmproject.com/)将P值或Z值转换为t值。随后,分别使用软件内置的“fMRI或PET”与“VBM-灰质”模板[30],按标准流程完成:(1)全局分析;(2)预处理;(3)均值分析;(4)阈值分析;(5)创建掩膜并提取结果。

       采用推荐参数:各向异性核函数,各向同性半高全宽(full width at half maximum, FWHM)为 20 mm,体素纳入概率阈值为P<0.005,峰值阈值Z>1,集群大小阈值≥10个体素,并使用1000次随机置换检验进行全脑FWE校正。该参数组合可在控制假阳性与保留统计效能之间取得良好平衡[30, 31, 32]。最终,所有TDACD与HC之间差异有统计学意义的脑区均通过MRIcron(v1.0.20190902)软件进行可视化呈现。所有脑区解剖分区均采用 Brainnetome Atlas与 Brodmann分区系统进行双重验证。

1.7 异质性和发表偏倚评估

       异质性通过Q统计量进行检验,采用I²值量化异质性程度;I2>50%提示存在中高度异质性。发表偏倚采用Egger检验进行定量评估,并绘制漏斗图直观展示[33]P<0.05提示存在显著发表偏倚[4]

1.8 敏感性分析

       使用SDM-PSI进行敏感性分析,以验证结果的可重复性[32]。经过预处理后,在主要分析中通过每次排除一项研究来进行,如果每次分析后获得的显著脑区都出现在之前所有或大部分显著脑区中,则结果是稳定和可重复的[34]

1.9 元回归分析

       使用SDM-PSI一般线性模型考察平均年龄、女性比例、受教育年限、糖尿病病程、BMI、HbA1c、MMSE与MoCA得分对TDACD患者GMV与功能活动异常的调节作用。为控制假阳性,分析采用严格阈值:空间平滑核FWHM为20 mm,体素水平显著性设定为P<0.005(未校正),并进一步结合簇水平FEW校正(P<0.05),峰值Z>1且簇大小不低于10个体素[35]。该阈值可在平衡敏感性与特异性的同时,有效降低假阳性结果的发生,是神经影像元回归回归分析的常用标准。

2 结果

2.1 文献检索与纳入研究特征

       通过系统检索共获得文献2659篇,其中FC研究1676篇,GMV研究983篇。剔除重复及不符合纳入标准的非研究型文献后,依据标题和摘要初筛。随后由两名研究者独立阅读剩余文献的全文,排除未涉及TDACD、未提供峰值坐标、缺乏认知评估、动物实验及仅ROI分析的研究。最终共纳入12项研究,其中7项FC研究[5, 18, 19, 36, 37, 38, 39]及5项GMV研究[11, 12, 15, 16, 40]图1)。表1列出TDACD组与HC组的人口统计学和临床特征,包括年龄、性别、受教育年限、糖尿病病程、BMI、HbA1c及认知水平得分。根据文献质量评估量表(总分20分),所有研究得分≥17分,表明纳入研究的方法学质量较高(图2)。

2.2 TDACD与HC之间的脑活动分析

       7项研究荟萃分析显示,与HC组相比,TDACD组在两处脑区的活动显著降低(表2图3):左侧缘上回(BA 40区,MNI坐标:-54,-54,28;SDM-Z=-4.678,P<0.000 5);右侧眶额回(BA 45区,MNI坐标:48, 40, -4;SDM-Z=-3.649,P<0.001)。异质性检验:左侧缘上回I2=72.4%,df=5,P=0.001;右侧眶额回I2=68.9%,df=5,P=0.004,均存在中高度异质性,可能与不同研究采用的功能连接分析方法(如度中心性、功能连接密度)、认知评估工具及患者病程差异有关。发表偏倚检验:Egger 检验显示左侧缘上回截距=-2.34,95% CI:-4.12~-0.56,P=0.018;右侧眶额回截距=-1.87,95% CI:-3.51~-0.23,P=0.029,均存在显著发表偏倚。漏斗图见图4。敏感性分析结果显示,左侧缘上回在7次重复分析中均保持显著,而右侧眶额回在7次重复分析中有5次结果保持显著。由于所有纳入研究的质量评分均≥17分(高质量研究),无法进行高质量与低质量研究的亚组分析,见表3

2.3 TDACD和HC之间的GMV分析

       5项GMV研究的荟萃分析表明,TDACD患者相较于HC组,在右侧豆状核,壳核(MNI坐标:34, -4, 4;SDM-Z=-2.933,P<0.01)GMV减少(表2图5)。异质性检验:右侧壳核I2=65.2%,df=3,P=0.022,存在中高度异质性,可能与不同研究的样本年龄分布、糖尿病病程及血糖控制水平差异有关。发表偏倚检验:Egger 检验显示右侧壳核截距=-1.62,95% CI:-3.15~-0.09,P=0.041,存在显著发表偏倚。漏斗图见图6。敏感性分析结果显示,右侧豆状核,壳核在5次重复分析中有5次结果保持显著。由于所有纳入研究的质量评分均≥17分(高质量研究),无法进行高质量与低质量研究的亚组分析,见表4

2.4 元回归分析结果

       基于7项功能研究的meta回归分析显示,年龄、女性比例、糖尿病病程、HbA1c、MMSE和MoCA对功能组和GMV组的主要结果没有显著影响,受教育年限与右侧眶额回脑活动改变之间存在显著关联:受教育程度越高,该脑区脑活动的减弱程度越小(图7)。

3 讨论

       本研究基于SDM-PSI,对灰质体积与静息态功能影像数据进行了meta分析,系统揭示了TDACD患者在结构与功能层面的脑异常。结果显示,TDACD组左缘上回(supramarginal gyrus, SMG)与右眶额回(inferior frontal gyrus orbital part, IFGorb)功能活动降低,右侧壳核(lenticular nucleus, putamen, PUT)GMV减少。敏感性分析结果稳定。多模态证据共同支持TDACD的核心病理特征为皮层—皮层下环路的协调受损,其表现为神经整合能力下降与认知控制效率减弱,为理解其神经机制提供了重要线索。

3.1 高级网络功能异常

       SMG位于顶叶下回,是额顶网络(fronto-parietal network, FPN)的关键节点,参与空间认知和情景记忆处理[41]。T2DM患者SMG区域功能架构稳定性下降,提示其功能活动异常可能损害相关的认知功能[42]

       IFGorb隶属前额叶并与默认网络(default-mode network, DMN)密切相关。GIRN等[43]指出,眶额及前颞叶皮层应被纳入DMN,该脑网络在认知与情感处理中扮演核心角色[44]。多项研究表明,T2DM患者的DMN功能障碍与工作记忆受损及情绪处理异常相关[45, 46, 47]。值得注意的是,T2DM相关的功能活动异常主要集中在DMN内部[18, 48, 49, 50]。YANG等[49]发现,在T2DM伴MCI的患者中,DMN与其他脑网络间的功能连接普遍减弱。DMN在静息态下是内部导向认知(如情景记忆、心理理论和自我参照加工)最活跃的脑系统之一[51, 52],维持其内部及与其他网络间的动态平衡对正常认知功能至关重要[44]

       本研究发现的SMG与IFGorb的功能活动降低,揭示FPN与DMN的节点异常及跨网络协同功能的失衡。这种失调可能导致TDACD患者在注意分配、执行功能及情绪调节等方面的复合性缺陷,从而构成其认知障碍的核心神经机制。

3.2 壳核萎缩

       TDACD患者PUT的GMV显著减少。PUT属于基底节结构的重要组成部分,长期以来被认为主要参与运动控制,但也参与词汇处理、语言产出和执行功能,其铁含量随年龄增长显著上升,可能影响多个认知领域[53]。该结构异常提示,TDACD患者的认知障碍可能并不仅限于皮层功能网络的紊乱,更涉及深部灰质区的形态学病变。

       与我们的结果一致,基于磁化率成像(quantitative susceptibility mapping, QSM)的研究进一步揭示了2型糖尿病患者PUT的微观结构改变。SUI等报道T2DM患者双侧壳核及尾状核的磁化率显著升高,提示局部铁沉积增加。该变化与MoCA得分呈负相关,表明壳核铁过载与认知灵活性及运动协调能力下降密切相关[54]。另一项QSM研究同样发现糖尿病患者PUT磁化率升高,推测铁稳态紊乱可能通过氧化应激和神经炎症介导结构损伤[55]。结合本研究结果与既往QSM研究证据,我们推测铁沉积可能导致壳核微结构损伤,长期进展可引起结构萎缩,最终导致认知下降。需要注意的是,本研究未直接测量脑铁含量,该推测需在未来研究中进一步验证。尽管 Egger 检验提示右侧壳核 GMV 结果存在一定发表偏倚,但敏感性分析显示该结果稳定,且与多项独立的脑铁沉积研究结果高度一致,提示该发现具有一定可靠性,仍需大样本多中心研究进一步验证。

       本研究发现TDACD患者同时存在右侧PUT的结构萎缩以及左侧SMG、右侧IFGorb的功能活动降低,这些脑区分别参与皮层下运动认知调控、FPN注意加工及DMN认知控制过程,提示 TDACD 患者可能存在皮层-皮层下环路的协同异常。ARNORIAGA- RODRÍGUEZ等[56]研究提示,肥胖相关的认知衰退主要与记忆相关脑区(如海马)的损害有关,常表现为决策、学习记忆和注意力等多方面的缺陷[57, 58, 59]。这些发现从反面印证,以PUT-SMG-IFGorb为枢纽的皮层—皮层下环路功能失调,很可能是TDACD更具特异性的神经标志。

       这些神经影像特征具有重要的转化价值,有助于建立更具鉴别力的疾病预测模型,为机器学习的早期诊断提供可靠的特征输入。同时,这些脑区,特别是SMG与IFGorb,作为认知相关脑网络的关键节点,已有系统评价和随机对照试验表明经颅磁刺激(transcranial magnetic stimulation, TMS)干预前额叶及顶叶脑区可显著改善认知障碍患者的执行功能与注意力[60, 61],其中针对TDACD患者的初步研究也显示出积极效果[60],提示这些脑区可能成为TDACD神经调控干预的潜在靶点,未来需开展大样本多中心临床试验进一步验证其有效性和安全性。

3.3 元回归分析结果

       本研究通过对多项人口统计学与临床变量进行元回归分析,发现除教育水平与右侧IFGorb功能活动存在显著相关外,其余变量均未对脑功能活动或GMV改变表现出显著影响。这一发现可从认知储备(cognitive reserve, CR)理论得到解释,这一理论认为,个体通过教育和持续的智力活动建立CR,这有助于减缓认知障碍的发展[62]。这一结果与T2DM领域既往研究基本一致。既往研究表明,较高认知储备水平与T2DM患者更好的整体认知表现相关,提示认知储备可能对糖尿病相关认知损伤具有一定保护作用[63]。因此,本研究中教育年限与右侧IFGorb功能活动的正相关,可能反映出教育所形成的CR对脑网络功能完整性具有一定保护作用。教育经验可能通过增强眶额叶相关网络连接及神经可塑性,提高脑网络功能韧性,从而在一定程度上缓冲糖尿病相关代谢异常对认知功能的影响。

3.4 与既往meta分析的比较

       既往已有多项meta分析探讨了TDACD患者的脑结构与功能改变。MA等[13]纳入了截至2021年的11项研究,发现TDACD患者存在广泛的灰质与白质异常;HE等[14]纳入了截至2024年的22项研究,发现TDACD患者在多个脑区存在结构与功能异常;DAI 等[28]纳入了截至2024年的15项研究,并进行了独立样本验证。本研究的增量贡献主要体现在:(1)纳入了最新发表的文献(截至2025年5月),包括2024年发表的3项高质量研究;(2)采用了最新的SDM-PSI 6.2.3版本软件,该版本在多重比较校正与异质性分析方面更为严谨;(3)首次系统探讨了教育年限等认知储备相关变量对TDACD 脑改变的调节作用。

       本研究结果与既往研究存在一定差异,主要表现为本研究仅发现3个脑区的显著异常,而既往研究发现了更广泛的脑区改变,这可能与本研究采用了更严格的纳入标准(仅纳入全脑分析且报告MNI坐标的研究)以及更严格的统计阈值(P<0.005,簇大小≥10个体素)有关。

3.5 局限性

       先前研究多报告颞上回GMV下降[7, 28, 35],而本研究未重复此发现,可能与以下因素有关。首先,本研究纳入的文献数量相对有限,尤其GMV研究仅纳入5项,统计检验效能可能不足;其次,不同研究所采用的影像学分析指标存在差异,本研究主要纳入VBM研究,未进一步整合皮层厚度、白质微结构等其他结构影像指标,可能导致结果间存在差异。

       此外,本研究仍存在以下局限性。首先,纳入研究间存在一定异质性,可能来源于受试者人群特征、疾病阶段、认知障碍严重程度、认知评估工具及MRI采集参数等方面的差异[58]。同时,功能研究采用了多种不同的rs-fMRI分析指标,包括功能连接、功能连接密度、区域同质性及度中心性等,而不同指标反映的脑功能特性并不完全一致,这可能是导致研究间异质性的重要原因之一。由于符合纳入标准的研究数量有限,本研究未能进一步开展亚组分析以探讨上述因素对结果的具体影响。

       其次,本研究采用基于坐标的SDM-PSI方法进行meta分析。尽管该方法能够较好地识别不同研究间一致性的异常脑区,但相较于基于原始统计图像的图像meta分析,仍可能存在一定程度的信息损失,无法充分利用全脑体素层面的完整统计信息。

       此外,部分早期纳入研究虽将受试者定义为“T2DM”人群,但其MoCA或MMSE评分已提示存在一定程度的认知功能下降。由于不同研究在认知筛查工具及认知障碍定义标准方面存在差异,部分“T2DM”组受试者可能已存在早期或隐匿性认知功能异常,从而增加研究间异质性,并可能影响灰质体积异常的空间分布结果。基于meta分析需尊重原始文献分组定义的原则,本研究仍按照原始研究分类进行纳入。

       最后,本研究纳入的GMV研究数量仍相对较少,可能限制结果的稳健性。同时,Egger检验提示功能分析中的SMG与IFGorb,以及结构分析中的PUT结果均可能存在发表偏倚,提示当前领域可能存在阳性结果优先发表的现象,从而导致部分效应量被高估。因此,对于相关脑区异常的解释仍需保持谨慎,未来仍需更多大样本、多中心及标准化研究进一步验证TDACD相关脑结构与功能异常的稳定性。

       综上所述,本研究基于SDM-PSI meta分析发现,TDACD患者在皮层与皮层下层面的协同异常。右侧PUT萎缩提示铁代谢与神经炎症共同参与其结构退变,左SMG与右IFGor功能活动下降则反映FPN与DMN间的整合失衡。教育水平与眶额叶活动的正相关进一步支持认知储备理论。总体而言,TDACD 患者存在皮层 —皮层下环路的结构与功能异常,尤其是以 PUT-SMG-IFGorb 为核心的相关脑区受损可能构成潜在的神经影像标志。这些结果为 TDACD 的病理机制、早期识别及神经调控干预提供重要线索。
